I know from my sixteen years in practice that the more information I was given on a new patient's Intake Form, the better prepared I was for their first appointment: it gave me the background I needed, alerted me to potential problems and allowed me to get to the issues more quickly. I've discovered that the same applies in the field of online marketing.

The more information you provide, the better the solution we will be able to provide. Ignore any questions that are not relevant: please give as much detail as possible to those that are.
